Role ScopeThe Clinical Services Manager (CSM) is a key leadership role within the Habitat Health PACE Center, responsible for overseeing the nursing and clinic teams to ensure highquality, participantcentered care. Reporting to the Center Executive Director, the CSM oversees clinic operations and outcomes while providing guidance and support to RNs in their case management responsibilities and their work as members of the interdisciplinary team (IDT). This role partners closely with the Center Manager/Director to ensure the overall success of the Center. Depending on Center census and structure, the CSM may also oversee additional members of the IDT. 

Core Responsibilities & Expectations for the Role 

  • Provide clinical leadership and oversight to members of the Interdisciplinary Team (IDT) and clinic staff, ensuring highquality, participantcentered care. 
  • Ensure clinical competency of direct reports through skills checks, coaching, mentoring, performance management, and ongoing professional development. 
  • Lead onboarding, training, and continuing education for nursing and clinic team members to maintain high standards of care and regulatory compliance. 
  • Monitor and ensure adherence to clinic and IDT service level agreements (SLAs), regulatory timelines, and documentation requirements. 
  • Oversee care transitions across settings (hospital, skilled nursing facility, home, etc.), coordinating with the clinical team, Medical Director, and other center leaders to ensure continuity and safety. 
  • Drive achievement of key performance indicators (KPIs) through effective planning, workflow optimization, participation in weekly tactical and monthly operating reviews, and provide support with quality initiatives for the Center. 
  • Maintain compliance with PACE, DHCS, and CMS regulations for audit readiness of the clinic, med room, and lab room; including audit preparation, monitoring areas of noncompliance, and implementing corrective action plans. 
  • Support clinic operations by addressing staffing needs, workflow issues, and operational barriers, escalating when appropriate. 
  • Provide direct clinical care as needed, including triage, emergency response, wound care, medication and vaccine administration, home care support, and participant education. 
  • Perform other related duties as assigned to ensure smooth clinic and Center operations. 

Required Qualifications:  

  • Active and unrestricted Registered Nurse (RN) license in the state of practice. 
  • Three (3) to five (5) years of clinical leadership experience, with demonstrated ability to lead teams and drive performance across multiple workflows. 
  • Proven ability to supervise and develop clinical staff, including nurses, medical assistants, and frontline supervisors. 
  • Strong decisionmaking and prioritization skills, with experience implementing scalable processes in a fastpaced clinical environment. 
  • Advanced communication and relationshipbuilding skills, with the ability to build trust, foster accountability, and collaborate effectively across interdisciplinary teams. 
  • Experience working with complex geriatric populations, including individuals with multiple chronic conditions, functional limitations, and behavioral health needs. 
  • Knowledge of clinical quality standards, regulatory requirements, and compliance expectations relevant to PACE or similar valuebased care models. 
  • Certified in CPR/BLS.

Compensation: 

The anticipated base salary range for this position is $123,000 -$145,000 and is bonus eligible. 

The final compensation offered will be determined based on factors including, but not limited to, the candidate's qualifications, skills, experience, education, certifications, geographical location, and business needs.
